Clinico-Pathological Studies on the Carcinoma of the Colon and Rectum




Abstract
With recent advances of cancer immunology especially in the field of malignancy in the gastrointestinal tract, various new concepts has introduced on the carcinoma of the colon and rectum and many of them has been accepted and attempted in clinical practice. In this regard, authors have reviewed 247 cases of the carcinoma of the colon and rectum which had been confirmed histologically and treated Surgically from Jan. 1958 to December 1971 at Severance Hospital Yonsei Medical Center. The objective of this paper is to evaluate the practical situation of carcinoma of the colon and rectum in Korea and to review foreign literature to remind the new concepts on the carcinoma of the colon and rectum. Carcinoma of colon and rectum seemed to be:increased in its incidence recently compare to the incidence of carcinoma of stomach in Korea., Among the 247 cases, the 175 cases (70%)As at step of our Department of Surgery for the investigation of the generalized peritonitis due to Salmonellosis, a clinical evaluation was made for the 69 patients admitted between January, 1969 and the end of 1971.

Followings are the results of this study.

1) The annual incidence is 23 cases, and this figure is not altered essentially since our first report in 1962.

2) There showed some tendency to occur in aged group (30% in 4th decade). 3) The incidence in male has risen up to 4 times of female.

4) The duration of ailment before perforation has somewhat prolonged (30 days or more is 17%). 5) 42 cases (61%) were operated within 24 hours after perforation, This fact contributes to the increased survival rate.

6) The overall mortality is 13%, this is a significant decrease.

were the carcinoma of the rectum and rest of them were distributed in the colon. Compared to former 8 years much progress has been approved in various points such as time of hospitalization and changes of misdiagnosis before confirmatory diagnosis and that much improvement in operability and resectability. In former period resectability was 58% but in later period resectability was increased up to 70. 1%. In the cases of carcinoma of the rectum, 89 cases were underwent Miles¢¥ operation and out of 14 cases who were underwent anterior resection 5 cases were reoperated due to local recurrence, so we are now insist strict indication for case selection for anterior resection.
